ComPacks is a New South Wales Health funded package program of care, designed to assist people at home by helping them regain independence following a hospital stay.
This program may include various types of care, such as domestic assistance or transport, for up to 6 weeks starting from the time of the patients discharge home from hospital.
The aim of this program is to prevent future hospital admission.
A Rapid Response Pack (RRP) is a one-off individual package of care for someone who is experiencing a rapid decline in function due to their condition. RRP funding is for direct service intervention (up to $2,000) or equipment (up to $5,000) that will enhance an individual’s capacity to remain at home and as independent as possible.
Community Options Australia has been selected by the Commonwealth Department of Health to administer the Continuity of Support (CoS) Programme for Individual Support Packages.
Continuity of Support (CoS) means assisting older Australians to achieve similar outcomes to those they were achieving prior to being transitioned to new funding arrangements.
Eligible clients will be transitioned to CoS at the time the National Disability Insurance Scheme (NDIS) is rolled out in their region. There will be no new entrants to the programme following the implementation of CoS in a region.
