Community and Home Care

Community Options Australia was selected by the Commonwealth Department of Health to administer the Continuity of Support (CoS) Program for Individual Support Packages.

Continuity of Support (CoS) means assisting older Australians to achieve similar outcomes to those they were achieving prior to being transitioned to new funding arrangements.

Eligible clients were transitioned to CoS at the time the National Disability Insurance Scheme (NDIS) was rolled out in their region.

There are no new entrants to the program following the implementation of CoS in a region.

Home Support Assessment

Community Options Australia, via its network of services, conducts home support assessments via the 'my aged care system'. These assessments are designed to engage with clients and identify their needs based on their social supports and home environment

Hospital to Home Transition Care

Community Options Australia coordinates several programs and service that assist clients to transition from hospital services to home care. this transition of care is crucial in supporting people who are vulnerable after a hospital stay and require additional support at home to assist their recovery and support reablement.

Community & Home Care

The Commonwealth Continuity of Support (CoS) program ensures ongoing support is received by older people with disability who are not eligible for the National Disability Insurance Scheme (NDIS)
