Guide to Best Practice – Coordinate Community Care

COA is currently working with 5 key agencies within our network to collaborate and develop a reference document associated with best practice in community care. COA is sourcing academic and research consultants to identify the requirements and resources to develop this guide.

A schedule of events and a consultation process will be developed and COA would like to thank all parties who are so far engaged in this project.
